진단명 : 정상 흉부 CT

건강한 성인 남자의 흉부 CT 영상 입니다. CT는 흉부 X선과 달리 단면영상으로 흉부는 조직의 대조해상도가 높아 적은 선량으로 영상을 얻을 수 있습니다. 이 영상은 3 mm의 단면 간격으로 얻어진 영상입니다.
CT는 보려고 하는 구조물에 따라 색조준위window level와 색조폭window width을 조절할 수 있으며, 종격동의 연부조직을 볼 때와 폐의 병변을 볼 때에는 window를 다르게 설정해야 합니다. 흉부 CT에서 보이는 정상 구조물들을 살펴보겠습니다.

먼저 연부조직이 잘 보이도록 설정한 영상에서는 (width 400, level 30) 혈관 및 종격동의 구조물들을 잘 관찰할 수 있습니다. 이 CT는 조영제를 사용하여 촬영된 영상으로, 혈관 구조물들이 조영증강되어 밝게 보이고 있습니다. 대동맥궁aortic arch에서 분지하는 동맥으로는 우상완두동맥right brachiocephalic artery, . 좌총경동맥left common carotid artery, 좌쇄골하동맥left subclavian artery가 있으며, 우상완두동맥은 다시 우총경동맥과 우쇄골하동맥으로 나뉩니다. 함께 주행하는 정맥으로는 좌, 우 쇄골하정맥, 내경정맥이 있으며 무명정맥으로 합쳐져 상대정맥으로 유입됩니다. 흉부 CT에서 관찰 가능한 주요 장기로는 갑상샘, 흉샘, 식도 등이 있으며 종격동의 림프절도 평가할 수 있습니다. 심장은 박동하는 장기로 심전도 동기electrocardiogram gating를 하지 않으면 정확한 평가가 어려우나, 좌심실left ventricle, 좌심방left atrium, 우심실right ventricle, 우심방right atrium을 대략적으로 구분할 수 있습니다.

폐를 잘 볼 수 있도록 window 를 조정하면 (width 1500, level -700) 폐의 병변을 잘 관찰할 수 있으며 기관지, 엽간열fissure 등의 구조물이 잘 보입니다. 기관지는 기관으로부터 우, 좌기관지로 나뉘며 우상엽, 우중엽, 우하엽, 좌상엽, 좌하엽 기관지로 다시 나누어집니다. 각 기관지는 분절segment에 따라 다시 나누어지며 분절의 구조는 다음과 같습니다.
- 우상엽: apical, anterior, posterior segments
- 우중엽: medial, lateral segments
- 우하엽: superior, anterior basal, medial basal, posterior basal, lateral basal
- 좌상엽: apicoposterior, anterior, superior lingular, inferior lingular segments
- 좌하엽: superior, anteromedial basal, posterior basal, lateral basal segments
엽간열은 일반적으로 우측 폐에 2개, 좌측 폐에 1개로 이루어지며 우상엽과 우중엽을 나누는 우소엽간열right minor fissure과 우상엽/우중엽을 우하엽과 나누는 우대엽간열right major fissure이 있습니다. 좌측 폐는 좌상엽과 좌하엽을 나누는 좌대엽간열left major fissure이 있습니다.
