주증상 : Left leg swelling
임상소견 : 내원 3일전부터 좌측 하지가 붓고 통증 발생
진단명 : 심부정맥 혈전증의 혈전용해술Thrombolysis of deep vein thrombosis

좌측 총장골정맥Left common iliac vein이 우측 총장골동맥right common iliac artery와 5번 요추체5th lumbar vertebral body사이에 눌려서 좌측하지left lower extremity의 정맥에 혈전증thrombosis가 발생하고 좌측하지부종left leg swelling과 통증이 동반되는 것을 장골정맥압박증후군May-Thurner syndrome (혹은 iliac vein compression syndrome) 이라고 합니다. 이런 경우 국소적 혈전 용해술catheter-directed thrombolysis 과 스텐트stent 삽입술을 시행하면 조기에 혈관의 재개통을 이룰 수 있고, 판막의 기능을 보전하여 만성정맥울혈증후군을 예방할 수 있습니다. 조영증강 CT에서 좌측 총장골정맥 내부가 혈전으로 가득 차서 조영증강이 되지 않는 것을 볼 수 있습니다. 혈관조영실에서 좌측 슬와정맥left popliteal vein을 천자puncture 하고 정맥조영술을 시행하였으며, 좌측 대퇴정맥left superficial femoral vein내부에 충만결손filling defect으로 보이는 혈전이 있었습니다. tPA(tissue plasminogen activator)를 이용하여 혈전용해술을 시행한 뒤 다시 시행한 혈관조영술에서 혈전이 모두 용해되어 슬와정맥에서 외장골정맥external iliac vein 은 재개통되었으나, 좌측 총장골정맥 이 압박을 받아 눌려 조영제가 차지 않는 소견이 보입니다. 혈관조영술 도중 좌측 총장골정맥에 도달한 조영제는 측부정맥collateral vein을 통하여 우측 총장골정맥 과 대정맥IVC 으로 흘러 들어가고 있습니다. 이러한 측부정맥순환은 좌측 하지 정맥혈류의 원활한 흐름을 방해하므로 좌측총장골정맥에 스텐트를 삽입하였고, 이후 좌측 외장골정맥 에서 대정맥으로의 혈류가 원활한 것을 볼 수 있습니다.
